This form must be filled out and signed by all Private Investors that you are dealing with.  The lender only needs to sign the form once, however, the form must accompany all private deals sent to head office for payment. 

Please send this completed form along with your Form 1 for signing by the Broker of Record This form is also to be included with all your files sent in for audit.  Failure to do so will result in delaying commissions paid.
